Submission Forms

"Please note:

From April 26 through May 2 the Virology Laboratory will not be working at full-staff capacity. We will suspend the rabies serology (RFFIT tests) during that time. We will resume normal operations starting May 3. You can mail your samples any time during that period, and they will be stored properly until May 3. "

We will resume normal operations starting May 27. You can mail your samples any time during that period, and they will be stored properly until May 27th.

Please complete one of the following forms, print it and mail it to the address listed along with your specimen. Please refer to this page for current pricing and specimen requirements.

If serum is required please do not send whole blood, or extremely hemolyzed and/or lipemic serum. When using serum separator tubes please centrifuge the blood after a clot forms, and transfer the serum into a separate tube. When multiple tests are requested on the same serum sample, 1 ml of serum is usually enough to run all of them.
