Therapeutic Drug Monitoring (TDM) Pricing and Sample Collection Protocols
Anti-epileptics
- Most tests (except cyclosporine) require 0.5 mL serum or plasma.
- Transfer separated serum or plasma to a plastic transport tube.
- Do not use a gel barrier tube. The use of gel-barrier tubes is not recommended due to the slow absorption of the drug by the gel. Depending on the specimen volume and storage times, the decrease in drug level due to absorption may affect a test result and interpretation.
- Please do not freeze samples.

Peak | Trough | Single | Peak & Trough
Bromide | 0.5 mL Serum /Plasma | BND | T, R | $55 | Gold chloride | No | Do not use a gel barrier tube.
Gabapentin | 0.5 mL Serum /Plasma | 2 hrs | BND Preferred sample if only one is submitted. | As received | $80 | $115 | Immunoassay | No | Do not use a gel barrier tube. |
Levetiracetam (Keppra®) | 0.5 mL Serum (NO plasma) | 2 hrs | BND Preferred sample if only one is submitted. | T, W, R, F | $80 | $115 | Immunoassay | No | Do not use a gel barrier tube. Please draw in a red top tube, allow time to clot, spin down, draw off, and send the serum. Only serum provides the cleanest sample and, therefore, the most accurate results. |
Phenobarbital | 0.5 mL Serum /Plasma | 2 hrs | BND Preferred sample if only one is submitted. | T, W, R, F | $35 | $50 | Immunoassay | No | Do not use a gel barrier tube. Fasting sample recommended |
Phenytoin (Dilantin®) | 0.5 mL Serum /Plasma | BND | Preferred sample if only one is submitted. | As received | $55 | $80 | Immunoassay | Yes | Do not use a gel barrier tube. Fasting sample recommended |
Valproic acid | 0.5 mL Serum /Plasma | 2 hrs | BND Preferred sample if only one is submitted. | As received | $55 | $80 | Immunoassay | Yes | Do not use a gel barrier tube. |
Zonisamide | 0.5 mL Serum /Plasma | 2 hrs | BND Preferred sample if only one is submitted. | T, R | $80 | $115 | Immunoassay | No | Do not use a gel barrier tube. Serum or plasma should be separated from red blood cells as soon as possible. Excessive hemolysis may interfere with the assay. |
